***IMPORTANT NEW 2015 TAX FORM***
Posted: November 19, 2015 Filed under: News Leave a comment »Please be aware that there is a new tax form required, starting with the 2015 tax return called a Form 1095-C. The Affordable Care Act(ACA) requires AT&T to provide a Form 1095-C to all employees, former employees and dependents with health coverage through AT&T. AT&T will send you a completed Form 1095-C in late January. You will need the information to complete your 2015 tax return. This is proof of health coverage which the ACA requires you to have or you will have to pay a tax penalty.
The 1095-C includes information on medical coverage received through AT&T during any portion of 2015. The AT&T Form 1095-C does not include information about medical coverage received through a third party, such as another employer or an insurer.
The form has three parts.
Part I provides basic information about you and AT&T.
Part II provides details about the type of health care coverage the company offered to you. The IRS will use this information to determine if AT&T met the employer mandate of the ACA.
Part III of the 1095-C provides information about you and your dependents who received AT&T health coverage during the year. The box will be checked for each month that you and each of your dependents were enrolled in health coverage for at least one day of that month. The IRS will use this information to determine if you or your dependent(s) would incur a tax penalty for failure to have minimum required health coverage.
