Once you have logged in make sure you review and complete the enrollment. <strong> The enrollment period will end on March 9, 2010 at 7:00pm. <\/strong>  The changes will take effect April 1, 2010.  You need to do this even if you did the annual enrollment in October.  <\/p>\n<p><strong>PAY CLOSE ATTENTION TO DEPENDENT COVERAGE AND VOLUNTARY PLANS<\/strong>(Careplus, Supplemental Life, Child Life Insurance, etc).  <strong>MOST EVERYONE IS SHOWN AS &#8220;SMOKERS&#8221; UNDER THE LIFE INSURANCE BENEFIT. <\/strong> Also the <strong>SPOUSAL &#038; CHILD LIFE INSURANCE<\/strong> has additional coverage options available.  Make sure <strong>ALL<\/strong> your dependents are listed.  Also check to see if your doctor is in &#8220;network&#8221;. (Go to www.myuhc.com).  Make the necessary changes.<\/p>\n<p>Below are some examples of the changes:<\/p>\n<p>1.  You MUST meet the annual deductible before the insurance pays anything.<br \/>\n2.  Once the annual deductible has been met you are responsible for 10% of the remain-<br \/>\n     ing fees until you have met the annual maximum out-of-pocket.<br \/>\n3.  Preventive in-network services paid at 100%<br \/>\n4.  No coverage for preventative services <strong>out-of-network.<strong><br \/>\n5.  After you meet your annual maximum out-of -pocket the plan pays 100%<br \/>\n     for in-network services.<br \/>\n6.  No deductible for prescriptions.<\/p>\n<p><\/strong>It is strongly recommended that once you have completed your enrollment that you print it for your records.<strong><\/p>\n<p>If you need assistance contact the AT&#038;T Benefits Center at 877 722-0020 between the hours of 7:00am and 7:00pm.
