The bargaining survey form should be completed by every<br \/>\nmember of your Local that is in the AT&#038;T SW bargaining unit.<br \/>\nYou can access the form either through the link or the attached<br \/>\nQR code. Feel free to share either or both via your website,<br \/>\npersonal emails from the Local to members, and any paper fliers<br \/>\npushed out to members:<\/p>\n<p>CWA\/AT&#038;T Bargaining Survey Form<br \/>\nQR Code &#8211; CWA\/AT&#038;T Bargaining Survey<\/p>\n<p>Additionally, the bargaining survey will require members to provide their up<br \/>\nto date home address, contact phone number, and contact email address.<br \/>\nThis information is very important and should be cross referenced to verify<br \/>\nUnion membership and update Aptify for future Mobilization.<\/p>\n<p>2. The bargaining request form should be completed by members<br \/>\nrequesting to make changes or add new language to the<br \/>\ncontract being bargained. There should be a new form submitted<br \/>\nfor each request being requested:<\/p>\n<p>CWA\/AT&#038;T Southwest Bargaining Proposal Request Form<br \/>\nQR Code &#8211; CWA\/AT&#038;T Southwest Bargaining Proposal Request Form<\/p>\n<p>The bargaining request form will require members to add their name,<br \/>\ncurrent job title, phone number, and personal email address to verify<br \/>\nmembership.
