Please submit only one proposal per form and write or print legibly filling out the forms <em>completely<\/em>.  Please limit your proposals to those you are willing to strike for.  If you have supporting documentation or information, please attach it to your proposal.  If you need additional proposal forms, please see your Strike Captain, or print from the attachment on this post.  These forms <strong>must<\/strong> be returned to your Strike Captain or Janice Bell no later than October 22, 2012.
